Student-Athlete Information

HIPPA PRIVACY REGUATIONS

HIPAA (Health Insurance Portability and Accountability Act) regulations require that all student athletes sign a medical information release form. The purpose of this form is to inform the community of newsworthy information concerning the health status of athletes as it related to team prospect and performance. All athletes have the right to revoke this consent at any time, and can do so by contacting the Sports Medicine Department.

ATHLETIC TRAINING ROOM LOCATION & HOURS

The main athletic training room is located on the first floor of the Miller Center (Room 137). The athletic training rooms are typically open Monday through Friday from 8:30am until 6:00pm. Saturday and Sunday hours are dependent on athletic schedules and by appointment only.

ATHLETIC TRAINING ROOM RULES AND EXPECTATIONS

  • 1. Student athletes are expected to report all injuries and illnesses to the staff within 24 hours.
    2. Report to the athletic training room daily until you are cleared by your athletic trainer.
    3. Be an active participant in your treatment and rehabilitation (Report any changes in your condition and ASK questions!!).
    4. All rehabilitation must be performed during the morning hours (prior to 12pm).
    5. Treatment is by permission of the Sports Medicine staff only-no self treatment with whirlpools, ultrasound or other modalities without prior approval.
    6. The Athletic Training room is not for social gatherings or a lounge-no loitering.
    7. Nothing is to be taken from the Athletic Training Room without permission of a staff member.
    8. Co-educational conditions exist in the Athletic training room-appropriate behavior, language and clothing are required at all times.
    9. No cleats may be worn in the Athletic Training room.
    10. No food or drinks are allowed in the Athletic Training room.
    11. No "OTC" (over the counter) medicine will be dispensed without approval from the staff.
    12. Coaches will be notified of any violations of these policies by the sports medicine staff.

SPORTS MEDICINE PHYSICIAN AVAILABLITY

Our team physicians are available on-call during the week. However, they do visit the athletic training room during the week. You must make an appointment through your athletic trainer in order to see these physicians during their weekly visit. However, they will take walk-ins on a first come, first serve basis after all appointments have been seen.

DENTAL SERVICES

The Department of Athletics will assume a degree of responsibility for payment in the case of dental damage incurred to teeth and other injures to the mouth only while participating in practice or intercollegiate game (match). Extraction of wisdom teeth, teeth cleaning, provisional filling of teeth or other dental work is not directly related to injury to the teeth that occurred during practice or competition is considered a non-athletic medical expense. Proper mouth guard should be worn during all practice and games.

OPTICAL SERVICES

The Department of Athletics may provide glasses, contact lenses or protective eye wear (goggles) for student-athletes who require visual correction in order to participate in intercollegiate athletics. Replacement for contact lenses will be provided in the event a lens is lost during a practice or game. All other losses must be the responsibility of the individual. Losses must be reported during the game, practice or immediately following the event if the Department of Athletics is to replace a lost or damaged lens.

INSURANCE COVERAGE

Wake Forest University Department of Athletics maintains "athletic-related" insurance on all intercollegiate athletic teams. This type of insurance is considered a secondary policy. This University policy was established to cover injuries sustained while participating in intercollegiate athletics. Additionally, the NCAA holds a catastrophic injury and disability policy on all student-athletes.

Like many other Universities within the NCAA, the Department of Athletics asks that the parents of student-athletes use their personal insurance as "primary" coverage for medical expenses. It is strongly recommended parents maintain medical coverage for their son or daughter to cover non-athletically related injuries or illnesses. There may be circumstances in which the Wake Forest University Department of Athletics is forbidden through the NCAA to provide assistance for medical conditions or accidents.

The Athletic Department assumes no financial responsibility for:

 Injuries or conditions received prior to the matriculation into Wake Forest University (i.e. pre-existing conditions or injuries).

 Injuries or conditions not incurred in a supervised practice or during intercollegiate contests (including injuries sustained while playing intramurals, inside your dormitory room and campus events, or injuries sustained while not enrolled in the academic semester).

 Any medical expenses to which you were not referred by a member of the Sports Medicine staff or Team Physician.

 Required or elective surgery where an injury or illness (received as a student-athlete at this University) is not the direct cause of athletic participation (i.e. appendectomy, bone tumor, wisdom tooth extraction, tonsillectomy, etc.).

 Injuries and illnesses that are not reported as noted above. Retroactive injuries occurring after graduation or departure from the University will not be considered.

STUDENT HEALTH SERVICES

The Student Health Center is available to all Wake Forest University student athletes 24 hours a day, 7 days a week (except during vacation periods). It is the responsibility of the student athlete to inform the sports medicine staff of any treatment or medications prescribed by the SHS as soon as possible. The Athletic Department recommends the use of the SHC when the athletic training room and team physicians are not available.

SUBSTANCE ABUSE EDUCATION AND SCREENING PROGRAM

The Wake Forest athletic administrative and coaching staffs condemn the use of any illegal drugs or the abuse of any substance that endangers the safety and health of its student-athletes. As per University policy, all intercollegiate student athletes will be subjected to year-round, random drug testing. The drug-testing program employed by the Department of Athletics gives you an additional reason to say "no" to any drug related temptations. Please refer to the Substance Abuse Education and Screening Program manual which is provided to you during your team's yearly NCAA certification meeting
